MN, OH, UT, VA and VT. Pharmacy discounts are not insurance and
are not intended as a substitute for insurance. MD, ND, OK, SC and SD
residents: Member shall receive a full refund of membership fees,
excluding registration fee, if membership is cancelled within the
first 30 days after receipt of membership materials. AR and TN residents: A refund of all fees will be
issued if membership is cancelled within the first 30 days. The
discount medical card program makes available, before purchase and
upon request, a list of program providers, including the name, city,
state, and specialty of each program provider located in the cardholder's

This plan provides discounts at certain healthcare providers for medical services.
This plan does not make payments directly to the providers of medical services.
The plan member is obligated to pay for all healthcare services but will receive
a discount from those healthcare providers who have contracted with the discount
plan organization.
In order to receive a discount you must pay your entire discounted
bill in full at the time of service or purchase. You are solely
responsible for payment of all fees due to provider. No portion
of any provider's fees or costs will be reimbursed or otherwise
paid by HealthCare Privileges™.
Hospital discounts are not available in Maryland. Emergency Travel
Assistance is not available to Florida, Washington, Connecticut
or Oregon residents. Physician, Hospital, Vision, Dental and Chiropractic
Benefits are not available to Vermont residents.
